Salem Academy Christian Schools

Salem Academy student-athletes are required to have the following forms completed and on file.  Forms are to be updated annually, with the exception of the Physical Exam Form which must be updated every two years.  Parents are encouraged to download the "Athletic Registration Checklist" to make sure they have everything need to receive athletic clearance.
